The study was conducted from July 2013 to January 2014in Tanza, Cavite to develop an official school website of Felipe Calderon Elementary School with student health monitoring record. Specifically, the study aimed to: Iplan a system that has a good data management and informative contents;2. analyze all the requirements needed to satisfy the expectation of the end-users; 3. design an official website that has user- friendly interface, with features to efficiently handle student annual health records; and 4. evaluate the system if it conforms to the desired output.

Several observations and interviews were conducted to determine the needs of the target end users categorized into: parent, admin and nurse. The researchers found out that there was a need for a good data management and distribution. The data gathered were analyzed to conceptualize a proper program flow. Using Notepad++as its platform for text editing, the conceptualized program was developed. The researchers used different programming and scripting languages HTML, CSS and Javascript to enhance the interface of the system. To establish a connection to the database and to restrict access of end users, PHP codes were used. In addition, the researchers incorporated a Content Management System (CMS) on the system to make it dynamic, convenient and easy to use.

There were a total of 200participants who evaluated the system consisting of 16 teachers (including the principal, nurse, ICT coordinators and guidance counselor), 128 parents and 56 guests. The system was tested and evaluated according to its accuracy, consistency, correctness, reliability, information on the program and user interaction. The technical aspect of the software and materials were also evaluated.

As a result, the system was in its "good" condition. The accuracy, consistency, correctness, reliability, information in the program, user interaction and technical aspect of the software and material was measured and checked.
